Additional HP is gained per Rest Phase. Amount of HP gained during Rest is determined by several factors. Stats and class are the most dominant. Make sure you have 18 Constitution before resting. Classes should use their main stats as a basis for HP gain. Strength and Constitution for Barbarians for example.

Additional HP is gained when you are in Cobrahn. Boone will give you +6 HP when you arrive. (If you did Boone during IEN, then do it again for +6 more HP!) Complete the Doom Town HP Quests with the HP Doc. The HP Doc will ask for a crit to be brung back to him or maybe a Tied item such as a Bone Ring or Flawless Ruby. There are several HP Docs in the tower. 19th, 20th and 21st level to gain entrance to the Docs. Also you must be enchanted by running the Pillar Room which in itself is dangerous.

Additional HP is gained by doing the 4 major HP Quests. Each of the Quests will gain you +100 HP. The Quests are: Ring Quest, Replacement Quest, Shackle Quest and Graagh Hally Quest.

Additional HP is added with the purchase of hit points by going to the HP Doctor. The amount of HP you can buy is limited by class. The actual cost of hit points is determined by how well the HP Doc likes you. The higher your Charisma is, the less you will have to pay. Having a CHR of 18 helps as well as the Indian Beads from the Thanksgiving Event. Wear Charisma rings if you have them. Smiling always helps.

Additional HP is gained by wearing/using items acquired during play. Rings, shields and other items can add temporary HP which when added to your base HP can cause the total to be greater.
Doing the right thing will make you strong.
Additional HP is gained by drinking Minotaur Bloods after Level 15 if the Blood is consumed on KM-5. A maximum of 20 additional HP is given by drinking.

Additional HP is gained by subscribing to the GOLD PLAN and Upper Guild Hall. Additional HP is also gained per month of GOLD PLAN. If you have been eaten or just had bad rests, then gain access to the Frore Clubhouse. With GOLD PLAN, Upper Guild Hall and Clubhouse, climb the stairs within and complete Item Quests or pay MEGS  to get back lost HP to max per level.
